Cardiovascular System:

The cardiovascular system bears the brunt of metabolic syndrome damage. Endothelial dysfunction is an early marker of cardiovascular damage in metabolic syndrome. The endothelium (inner blood vessel lining) becomes inflamed and less able to produce nitric oxide, a substance that helps blood vessels relax and maintains vascular health. This dysfunction occurs before visible plaque buildup and is reversible with appropriate treatment.

Pancreas and Glucose Metabolism:

The pancreas, located behind the stomach, plays a central role in metabolic syndrome through its endocrine function. Islets of Langerhans within the pancreas produce insulin (by beta cells) and glucagon (by alpha cells). In metabolic syndrome, chronic insulin resistance places increasing demands on beta cells to produce more insulin.

Initially, beta cells adapt by increasing insulin production—this is why blood glucose may remain normal in early metabolic syndrome despite insulin resistance. However, over time, this compensatory mechanism can fail. Beta cells become exhausted and may undergo apoptosis (cell death), leading to progressively impaired glucose regulation and eventually type 2 diabetes.

The progression from normal glucose tolerance to prediabetes to type 2 diabetes often occurs over 5-10 years, providing a critical window for intervention. Even after diabetes develops, managing insulin resistance remains important for controlling blood sugar and preventing complications.

Liver and Metabolic Processing:

The liver is central to metabolic syndrome pathophysiology. It receives blood directly from the intestines and pancreas via the portal vein, exposing it to high levels of nutrients and insulin after meals. Visceral fat releases free fatty acids into this portal circulation, flooding the liver with substrates for metabolism.

The liver responds by increasing glucose production (gluconeogenesis) even when blood glucose is already elevated—the normal suppression of glucose production by insulin is lost. The liver also produces more VLDL (very-low-density lipoprotein) cholesterol, contributing to dyslipidemia. Other Organ Systems:

Diagnostic Criteria by Organization

Metabolic syndrome is diagnosed using standardized criteria established by several international organizations. While the core components are similar, there are some differences in specific thresholds and requirements.

International Diabetes Federation (IDF) Criteria:

NCEP-ATP III Criteria:

The National Cholesterol Education Program Adult Treatment Panel III defines metabolic syndrome as having three or more of the following: waist circumference >102 cm (men) or >88 cm (women), triglycerides >=150 mg/dL, HDL cholesterol <40 mg/dL (men) or <50 mg/dL (women), blood pressure >=130/85 mmHg, or fasting glucose >=100 mg/dL. This definition does not require central obesity as an mandatory component.

World Health Organization (WHO) Criteria:

The WHO requires evidence of insulin resistance (impaired glucose tolerance, diabetes, or insulin resistance measured by euglycemic clamp) plus two of the following: BMI >30 kg/m² or waist/hip ratio >0.9 (men) or >0.85 (women), triglycerides >=150 mg/dL or HDL cholesterol <35 mg/dL (men) or <40 mg/dL (women), blood pressure >=140/90 mmHg or on antihypertensive treatment, or microalbuminuria (urinary albumin excretion).
